Routine appointments
To request a routine appointment in the next 7 days
- use our appointment request form, Monday to Friday from 8am to 6:30pm. We will respond within 1 working day during opening times
- phone us on 0114 263 6100, Monday to Friday, when we are open

Your appointment
However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email.
Appointments by phone, video call or by text or email can be more flexible and often means the doctor or nurse can help you sooner.
Sign up for SMS reminders
We have a texting service which allows you to receive confirmation and reminders about your appointments.
To have this service you will need to register by completing a consent form.
Please bear in mind, we do not send text confirmations or reminders for TELEPHONE appointments.
Please remember to update your contact details with us when you change address, telephone numbers and email address.
Cancelling or changing an appointment
To cancel your appointment:
- use your NHS account (on the NHS website or in the NHS app)
- fill in an online form – only if it is more than 24 hours in advance
- reply CANCEL to your appointment reminder text message
- phone us on 0114 263 6100, Monday to Friday, when we are open if your appointment is less than 24 hours away
- visit the surgery and speak with a receptionist, Monday to Friday, when we are open
Extended access services
We offer extended access remote and face to face appointments at our Porter Brook site on the following days:
- 7am to 10am Monday, Wednesday, Thursday, and Friday
- 5pm to 8pm Wednesday
If there is limited availability for appointments available during the usual surgery hours you may be offered an appointment at an external hub outside of core hours.
Remote and face-to-face appointments can be made with a variety of GP practice staff including Doctors, Advance Care Practitioners, Nurses, Nurse Associates, Health Care Assistants, Physiotherapists and Pharmacists.
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Please tell us:
- If there’s a specific doctor, nurse or other health professional you would prefer to respond.
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
Home visits
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need. To request a home visit, it’s helpful if you phone the practice before 10am. You can also:
